Obligatory Legal Provisions and Client Consent

Clients must accept the following terms as provided prior to scheduling an appointment.

In order to enter into a contractual agreement for coaching services with Cindy Burrell (Cindy), the client must read the following statements and agree to the policies and procedures of this service before placing the call.

Any other party whom Client may wish to include in their counseling session are similarly obligated to comply with the policies. The
additional party is required to contact Cindy before being allowed to participate in the client’s sessions, and Cindy is at liberty to approve or decline the request at will.  Client agrees to waive confidentiality protocols, if any other party, including a family member, participates at his or her request in personal coaching  session(s). There are no
exceptions to this rule.

I understand this is a faith-based, Christian coaching service.  Client confidentiality will be respected unless Client agrees that outside
intervention or input is available and deemed mutually appropriate .

Cindy Burrell offers coaching based upon her knowledge and
experience with regard to abuse and the abuse dynamic.  The client acknowledges that Cindy Burrell is not a licensed  counselor or
psychologist.   In the unlikely event of subpoena, or litigation, Cindy intends to exercise her right to fully invoke the client confidentiality privilege for the sole purpose of protecting her position as a personal coach acting in that capacity  and in defense of the sacred trust of those with whom she consults.

The client understands that no guarantees or commitments of any kind have been offered by Cindy with regard to addressing or
resolving personal experiences as disclosed, or the possible results or preferred outcomes of coaching.

The client agrees and understands that full payment is required prior to the consultation unless other arrangements have been agreed upon.   The client  accepts responsibility for any legal or collection fees related to nonpayment, including fees for insufficient funds.

Client accepts full responsibility for charges for scheduled consultations, including those with dependent children.  Client understands any threats of imminent harm to self, or others, including but not
limited to, child molestation/abuse, and/or elder molestation/abuse, must be reported by Cindy to the proper authorities.

The client releases from all liability, in any form, that may be charged against Cindy by Client, his or her family members or estate for
actions concerning coaching.  Cindy Burrell shall not be liable for any damages or injury arising out of the session.  Cindy Burrell disclaims any and all liability for direct, indirect, incidental, consequential, punitive, and special or other damages, lost opportunities, lost profit or any other loss or damages of any kind.

Client enters into this agreement being of sound mind.

When Client calls the number provided, it affirms that Client has read, understands and accepts all provisions.

Appointments will be scheduled with the mutual consent of Client and the consultant on a per-minute basis. Coaching that extends
beyond the scheduled appointment will be billed on a per-minute
basis at the same rate and with the mutual consent of Cindy and Client.

Any conversation that Cindy deems offensive, hostile, or in any way inappropriate will be immediately discontinued, Client will be billed for the time used accordingly, and Client forfeits any future
opportunity to communicate with Cindy Burrell.   Client may
discontinue the consultation at any time and will be billed for the time utilized.

Failure by Client to contact Cindy at the scheduled time, but makes contact within 15 minutes of the scheduled time agrees to
forfeit the lost time accordingly unless other arrangements are made and agreed upon by Cindy.  Failure to contact Cindy more than 15 minutes after the scheduled time will result in cancellation of the
appointment and billing of the client for the time forfeited.

Cindy Burrell reserves the right to refuse service to anyone for any reason at any time.

Rates and Payment

To schedule a safe, private consultation with Cindy by phone, the hourly rate is $60.00 per hour ($1 per minute), and payment is required in advance.

If we are unable to connect, payment will be refunded.
